Mortgage Field Service Application 1:

Complete the one page application entitled (National Assoc. Mortgage Field Services, Inc.) if the following applies:
- Your gross sales are under $25,000, and
- You only want preservation work or inspections you do for NAMFS members covered/insured.
"Note: This is a group policy that runs from January 1st to December 31st. $771.00 minimum premium applies until June 30th. $513.50 minimum premium applies if you sign up between July 1st to September 30th. $359.00 minimum premium applies if you sign up between October 1st to December 31st. These premiums include a 3% tax and a $50 fee."

- Your gross sales exceed $25,000.
- You want all your field service work covered/insured whether or not the company hiring you is a member of NAMFS.
